Is my MOSFET bad?

Pin 4: 0.03v
Pin 5: 12.6v

I'm not getting 12.6v on the source.
Is it because my drain is less than threshold voltage?

This is from my 820-2879-B board. I'm trying to figure out why my PPBUS_G3H is only 0.03v.
