Replace heat sink (or add fan) tips?

I've seen several posts advising replacing the heat sink on various boards (in my case it's for the main board on a 60pv450 lg plasma (attached). Any general advice on where to get heat sinks that are improvements over the old one? Last thing I want to do is make it worse than before--is "bigger" all I'm trying to go with here? Also--is putting a fan in worth the effort--and any advice on what kind of fan and how to attach / wire it in?

First is the heat sink really hot when the board is in use?

it's a good looking sink.

is it glued on, or held with tape?
if tape then step1 is to remove it, clean it and refit with thermal glue.

because the heatsink must take the heat from the chip.
then it must transfer it to the air - so is there airflow?

My advise follow what Stj said, replace the thermal pad with thermal glue. Then add a small fan over it.
