Thanks for the follow up, glad to hear that it just the bad diode, it took me a long time to narrow it down to that D200.

what does that D200 actually do in that circuit? If you don't mind explaining...

Pin 3 of the IC is the Over voltage sensing pin, the bad diode has small leakage current and cause the IC to trip.
That is why when d200 is removed, the circuit works fine (but without OV protection).
