Can we see the bottom side of the board to see how it is connected? It looks like inrush current limiter.

OK, it is what I thought what it is, it is Inrush current limiter, the Line (Hot) voltage (black wire) is fed into one leg, output is fed to the AC input of the bridge. Are you able to take the resistance reading on its two legs? What kind of product is it.
You may have shorted load that is why it is damaged like that.Never stop learning

It's from the motor on a clothes turn wheel like at a dry cleaner. I'll try to check the resistance tomorrow but I can't test anything inline as its from my friends dry cleaning business so he old have to shit down while we put it in. Thanks for the help!

I do not see 12 Ohms, you may be able to use 10 Ohms instead

I would get both,
The 10 Ohms will limit the max current to 12A in-rush.
The 14 Ohms will limit the max current to about 8.6A in-rush.
We do not know what the load running current is, so we need to measure the voltage drops on them to see how much it will have to dissipate the power drop once it is heat up (resistance will drop to low value once the current start flowing through the NTC).
