Very, very rare. Exceptions apply for the high voltage inverter transformers, which generally fail due to intra-winding insulation failure (due to the high intra-winding voltages.)
A work colleague of mine told me that they didn't like to have transformers that bridge the primary-secondary barrier fail, for any reason, because it could compromise the isolation safety of the equipment. There's only a few layers of insulating tape between the copper windings in a transformer. A flash over or arc inside it could burn through that insulation, creating an isolation fault and an electrocution hazard.Please do not PM me with questions! Questions via PM will not be answered.
