ATI Radeon X800 XT PE AGP

    Rec'd this card - dead.

In fact, most of the SMT poly caps topside had been removed.
As it appears all of these have been removed with force,
two of the caps have lifted the foil on the pcb.

This isn't so bad - I can pop in some Pana FM to replace
these in order to test go/no go.

What I don't have (as it was missing) was the powerlead for
the heatsink fan. I would like to ID this in order to obtain.

This is a 3 pin type.
The HSF end has a smaller connector than the board end.
In common with these types of connectors, they are keyed
to go in one way only.

    Can any members help?

It's a standard fan connector. One of the pins is to measure the RPM and the other two are standard. Take any VGA fan or an old 486 CPU fan and connect it somehow. No need to start purchasing rare parts.

Just solder the leads from the new cooling fan directly onto the board. Or power it externally from a molex connector on the PSU. Connect the fans positive wire to the yellow wire (+12V) the remaining wire to either of the black wires (ground) in the center. If it's a 5 volt fan connect the positive lead to the red wire (+5V).
Elements of the past and the future combining to make something not quite as good as either.

        Can't do that.

This is a non-standard proprietary ATI all-copper HSF.
The fan end is socketed as well as the board end.

I tried a WOL connector but this does not have the
correct connector to mate to the HSF albeit it is
        a three pin type.

        Thanks

Force it if the pinholes are too small. I have done this before.
