Here's the BIOS backup. I uploaded in the original post but it doesn't appear for some reason.
I'll update with the password screen shortly.

      Edit: Attached password screen

archive it with winrar or 7 zip and upload it here....don't post pictures unless you are asked to..
Due to a lack of donations, server free space at a critical level, and possible closure of Bios Requests all donations are welcome, click:

Every donation made will go towards server fees and maintenance costs.

I've seen other topics that required photos and I thought to make it simple from the start.
As a side note, the reply utility doesn't allow uploading of 7z archives.
          Attached it as RAR now.

          Thanks!

              BIOS chip: Winbond 25Q16JWIQ
              Programmer used: CH341A
I know it's a simple device, but it worked for all the devices I used it on until now.
I can read it again, no problem.
Anything I can check to make sure it's good?

              Thanks!

the chip is not what you need..look for another chip..128 or 256 write on it..i don't know what chip you have there..and put the content here..

I got confused by reading another topic, I read the GPU vBIOS instead of the actual BIOS. Will get back once I have a dump of the right one.
