I was able to fix the issue by extracting the entire Gbe region and reflashing.

I think I may have messed something up. I used a hex editor to modify the MAC address at 1000-1005 but I'm getting PXE and NVM errors. I attached the PXE error. After the PXE error I can boot to the OS and the new MAC is recognized and Intel ATM is working. I was able to disable PXE boot to make the error go away. I'm getting an invalid NVM checksum error when booting the Debian installer.
I'll try extracting the entire Gbe region like SMDFlea suggested and reflash....

Changing MAC address in ThinkCentre M910q BIOS
I have two Lenovo ThinkCentre mini I flashed the same .bin file on both devices and I noticed that the MAC address is the same for both devices. I plan on using Intel AMT on these devices, so the MAC will need to be changed at the BIOS level not the OS level. Is there a way to do that with a tool, or is modifying the .bin file the only way?

bump. I found a BIOS from another thread with the same motherboard model and tried cleaning the ME and copying the DMI from the original BIOS but now the SATA SSD isn't being detected and it still shows the model/SN of the BIOS I downloaded from another thread.

The replacement power supply did not fix my problem, and I can't explain that. When I saw the YouTube video with the exact same symptoms I figured I would give it a shot and it worked in my scenario. The set is still running great to this day....

The power supply readings are still fluctuating with just the power supply and main boards plugged in....

With the main board disconnected, Pins 3, 5, and 7 measure a steady 19V....

Both power supplies read 19V on +12V_A when either power supply runs solo. Furthermore, both power supplies read between 6V~13V when the old or new main board is connected. This leads me to believe the issue is not power supply or main board related or I'm not getting a good reading from the power supply when the main board is connected. If I put a meter on the main board, I get the same fluctuation from the connection coming from the power supply 6V~13V.

Yes its set to DC V. I tested the outlet using AC V.

It's a brand new unit and battery. I tested on an outlet and its flickering between 122.6 and 122.7 volts
